1. 12th or equivalent in the science stream from one of the recognized boards; Physics, Chemistry, and Biology with English as main subjects. Maths can be either optional or with provisions.
2. 50-60% is the minimum aggregate marks considered by most of the colleges whereas entrance exam is the gateway to study in particular institute. Many institutes could lower the boundary of 5 percent marks barrier for student from reserved category.
3. You need to qualify an entrance exam that weighs according to the colleges/universities you are applying to. It may be JEE Main, NEET, AIIMS, or some state level exam.
4. The least age for most of the admissions would be 17 years as on the last day of December of submission year. Could be another factor taken into account.
5. Medical certificate which should come with mentioned eyesight quality, no squint, color blindness etc. is required.
6. A clear interview round is expected if there are any norms related to that.
7. Most of the institutions prefer you have copied these subjects if you have studied group PCB in 12th instead of Mathematics.
